The performance summary card displays key information about a group child care program. All group child care centers must display a performance summary card issued to them by the NYC Health Department.

Only child care centers issued permits by the NYC Health Department will receive and are required to post a performance summary card. Home-based programs and school-based programs will not receive this card.

Group child care centers must post the card at the entrance of their program. The NYC Health Department is educating child care programs on how to comply and may issue a summons for violations, subject to a $500 fine, beginning on July 1, 2026.

Beginning August 15, 2025, centers can:

  • Request a replacement card if their card is damaged or defaced
  • Request an updated card because their information has changed

If your child's center has not posted a card, after September 8, 2025, you should file a complaint with 311. The card must be at the entrance of the child care center.
